Cross-Cultural Psychoanalytic Practices in Cases
of a Chinese and an Italian Patient: Two Case Scenarios.

Come
esperienze cliniche che interessano contesti culturali diversi possono aiutarci
nell’approfondire le nostre conoscenze riguardo il processo ed il trattamento
psicoanalitico? Questo è un po’ l’interrogativo di fondo del presente lavoro.
Lavoro che coinvolge due soggetti in analisi, uno di origine cinese, che vive
negli Stati Uniti da un po’ di tempo, l’altro che è nato e vive in Italia.
Anche riguardo le situazioni cliniche presentate si è cercato di mantenere
l’aspetto interlocutorio offerto da una visione transculturale.
Abbiamo osservato come alcune caratteristiche si propongano in entrambe le
situazioni cliniche e come altre sembrino invece più rappresentative di loro
differenze. Abbiamo inoltre ipotizzato alcuni fattori come un "comune
denominatore transculturale" del processo psicoanalitico stesso. In
particolare nel contesto del paziente cinese, si è evidenziata una certa non
familiarità con lo strumento analitico, assieme ad un’ attitudine più rivolta
verso un rapporto educativo-prescrittivo. Questi ostacoli inevitabilmente
portano ad una certa difficoltà ad aderire all’analisi ed al potersi lasciare
andare al processo associativo di questa.
Si è anche però notato come il "sogno" possa essere un elemento
praticabile, in analogia con il pensiero inconscio e quindi utilizzabile come
via per avvicinare ‘stratificazioni’ più profonde.
Questo studio ha rappresentato un utile aiuto nella comprensione della
complessità della mente, attraverso le differenze culturali. Ma ha anche
fornito un tentativo di aprire nuovi approcci clinici individuali, attraverso
l’uso di esempi e stimoli provenienti da altre culture.
Ci è sembrato particolarmente utile poter soffermarci su queste diverse
situazioni cliniche, considerando come lo studio di diverse ‘geografie’,
rappresenti un valido aiuto alla comprensione delle complesse ‘geografie della
mente’, in un mondo che tenderebbe sempre più ad appiattirle. In questo appiattimento,
infatti, si rischia di cadere nello stesso inganno che ci potrebbe essere nel
confondere una mappa del mondo, con il mondo stesso.

Abstract
How can clinical experiences from different cultural fields help us to deepen
our knowledge about psychoanalytic processes and treatments? This is the
fundamental question of this paper. Our work involves two subjects of
psychoanalysis, one a person of Chinese origin who has been living in the US
for some time, the other an individual born and living in Italy. With respect
to the clinical situations presented, this study attempts to maintain the
discursive aspect offered from a transcultural point of view.
We observed how some characteristics were shared between the subjects and
others seemed to represent differences in their clinical situations. We have
hypothesised some factors, such as transcultural common denominators of the
psychoanalytic process itself. In particular, in the Chinese patient’s context,
we found an unfamiliarity with the nature of psychoanalysis together with an
aptitude more common to a prescriptive and educative relationship. These
obstacles inevitably lead to a certain difficulty in adhering to the free
associative process of the psychoanalytic treatment.
In other ways, we found dreams to be a useful element in analogy with
unconscious thought and therefore a path we could use to further the
investigation.
This study is a useful aid in understanding the complexities of the mind –
through transcultural differences – but it also attempts to open a new clinical
avenue by using the examples of other cultures to increase our awareness and
ability to improve clinical approaches to our work. We found it particularly
useful to examine these different clinical situations by thinking about how the
study of different geographies helps our comprehension of the complex geography
of the mind in a world which should tend to flatten them. In this flattening of
the geographies, we risk confusing the interpretation of a flat map with the
real world itself.

Adolescenza, Isteria di transizione e processo di soggettivizzazione

La
trasformazione del corpo al momento della pubertà impone all’individuo un
lavoro di ridefinizione e di creazione di nuove rappresentazioni di sé: è il
processo psicologico dell’adolescenza. E’ per questo di grande importanza
osservare i fenomeni di natura isterica che si verificano nel corso di questo
processo evolutivo facendo ricorso alla teoria psicoanalitica dell’isteria che
da un lato è una teoria dell’unità psicosomatica ed un modo per pensarla e
dall’altro è una teoria dei processi di simbolizzazione.
I sintomi isterici sono in questi casi un tentativo per effettuare la
transizione verso una rappresentazione di sé che preveda l’integrazione del
corpo sessuato.
Nella comunicazione cerco di evidenziare, con delle esemplificazioni cliniche,
alcune indicazioni tecniche e presentare qualche osservazione sul piano
teorico.
Il corpo, in quanto dato, può essere percepito con un senso di estraneità e
minaccia dall’Io in quanto presenza e testimonianza di un ‘altro’ estraneo ed
intollerabile.
E’ lo stesso vissuto talvolta descritto dai pazienti nei confronti dei sogni.
Pensare il proprio corpo ed il proprio sogno sono opportunità fondamentali
perché l’individuo possa riconoscere il modo articolato e complesso in cui è
fatto: Caratteristico della psicoanalisi è un modo di pensare l’individuo
soggetto come esito di un processo di riconoscimento della molteplicità e
relatività dei soggetti che ciascuno ha in sé, cioè di un processo di
soggettivizzazione; una concezione che può essere ritenuta implicita nella
concettualizzazione dell’apparato psichico sviluppata dalla psicoanalisi e che
con essa ritengo debba costantemente articolarsi.

Adolescence, Transitional Hysteria, and the Process of Subjectivisation

Bodily transformation at the moment of puberty requires the individual to
undertake the work of self-redefinition and of creative self-representation:
this is the psychological task of adolescence. Therefore, it is very important
to observe phenomena of a hysterical nature that can occur in the course of
this developmental process, drawing on the psychoanalytic theory of hysteria
that, on the one hand, is a theory of psychosomatic unity and a way to think of
it, and on the other hand, is a theory of processes of symbolization.
Hysterical symptoms are in these cases an attempt to carry out the transition
to a self-representation that anticipates integration of the sexualized body.
It is a form of necessary and transitory hysteria: a transitional hysteria.
In this paper, I will try to highlight, with clinical examples, some technical
indications and to present some observations on the theoretical level.
The body as given can it be perceived with a sense of extraneousness and threat
from the Ego in its role as the presence and testimony of an alien and
intolerable "other." It is the same experience sometimes described by
patients in relation to dreams.
To carefully consider one’s own body and one’s own dream are fundamental
opportunities because the individual can recognize the articulated and complex
way in which he is made: characteristic of psychoanalysis, in fact, is a way of
conceiving of the individual subject as the result of a process of recognition
of the multiplicity and relativity of subjects that everyone has in himself,
that is, of a process of subjectivisation; a conception that can be considered
implicit in the conceptualization of the psychic apparatus developed by
psychoanalysis, and that I consider must itself be constantly articulated.

Presentazione :Italian Psychoanalytic Annual – Rivista di Psicoanalisi

One of the purpose of our meeting here in Beijing is to offer the
possibility to meet psychoanalysis in its different ways of expression. This is
why the Italian Psychoanalytic Society have decided to present you our
editorial initiatives. We think that in different places, according the
specific history, culture and traditions people have and had there,
psychoanalysis can develop a specific way to deal with clinical and also with
theoretical problems. In Italy we come from an history of very important and
different experience of meeting psychoanalysis, that is psychoanalysts, coming
from different countries. We think that these exchanges leaded us to develop a
peculiar panorama that we wish to present and let know beyond the linguistic
barriers that prevent the most part of colleagues in the world to reach our
psychoanalytic production.
This is why since 2007 we publish each year The Italian Psychoanalytic Annual
in English language. It’s a selection of papers published during the previous
year in Rivista di Psicoanalisi, the journal of the Italian Psychoanalytic
Society. Our aim is to adequately represent its polyphonic nature and, in the
variety of tones which characterises it, to introduce the harmony and
originality of its voice.
Our Society is the oldest and most numerous society in Italy: it was founded in
1932 and the same year was founded our Rivista di Psicoanalisi.
Now in Italy we have ten Centres, each of which both organises its own
activities and participate to the common cultural activity of the Italian
Society.
We have made a habit of setting up meetings with psychoanalysts from different
countries so that their thoughts have been shared with those we developed in
our Society. This habit goes some way to explaining its polyphonic features.
The scientific activity carried out at national level made up of frequent and
systematic exchanges, have over time enabled and facilitated, the harmonization
of the multiplicity of voices, favouring creative integration and new
modulations. Which is to say that our Annual will therefore speak English…
with a marked as well as, we think, a recognisable and distinctive Italian
accent.
The Rivista di Psicoanalisi, the official organ of the Italian Psychoanalytic
Society, does not have issues devoted to specific subjects, but in principle
accepts contributions spontaneously submitted for publication by authors, these
being peer-reviewed in accordance with the relevant international standards. In
this way it can better reflect the plurality of theoretical currents and
references represented in our psychoanalytic society, the common ground shared
by its members and the various matters addressed in ongoing individual and
group-based research.
The titles of each volume of the Annual, as well as those of the quarterly
issues of the Rivista di Psicoanalisi, are specifically intended to draw
attention to some of the threads that we have a diverse range of contributions
into a possible common fabric.
The first issue of the Annual, as I said, was edited in 2007.
Titled «Freud after all», this issue presents the papers that Rivista di
Psicoanalisi dedicated to celebrate the 150th anniversary of Freud’s birth and
documents how we can reading again and thinking again his work.
«Theory and observation: redefining the field» is the title of the second
issue.
To redefine the field is a standard procedure in science and the contributions
in this volume of the Annual likewise identify some of the cornerstones of
psychoanalysis that must be present if it is to thrive in the contemporary
situation.
Hence this volume can be seen at one and the same time as continuous and
discontinuous with its predecessor, «Freud after all».
The third volume, the 2009 issue, is titled «Our identities in chiaroscuro».
I underline the plural: identities. Our psychoanalytic identities are plural
and Rivista di Psicoanalisi has set itself to facilitate dialogue among this
plurality so as to ensure that the meeting is fruitful, within the rigours of
the method.
The more recent issue is that of this year, titled «Explorations of the
unconscious: clinical perspectives».
For psychoanalysis, the unconscious is the inescapable link between clinical
and theoretical experience, as well as the basis of the latter’s potential
contribution to the understanding of the profound social and cultural
transformations now emerging and of the consequent collective and individual
forms of malaise.
So we have the unconscious and our problem to think it, yet again.
As you can see in the paper we’ve distributed, both the Rivista di Psicoanalisi
and The Italian Psychoanalytic Annual can be founded in the PEP-Web and in the
PEP DVD Archive.
Some copies of them can be seen here and if somebody is interested please
contact me during these days or write to our editorial board.
Thanking you for your kind attention, I bring you also greetings of our present
editor, Alberto Luchetti, and of our president Stefano Bolognini that couldn’t

"In
credito con la vita"
Riflessioni sul trattamento della patologia del carattere collegabile a
protratte sofferenze infantili.

A partire dalla lettura di alcuni casi clinici freudiani ( l’"Uomo dei
lupi", Riccardo III, il Kaiser Guglielmo II) ho tratto indicazioni utili
sia per il trattamento delle dinamiche transferali e controtransferali che si
sviluppano durante l’analisi di pazienti con difficoltà caratteriali
collegabili a sofferenze e/o anomalie infantili dovute a malattie fisiche o a
situazioni ambientali sfavorevoli sia, più in generale, per la comprensione
della patologia del carattere.
Essere consapevole dei propri affetti di vergogna ed invidia collegati a
situazioni infantili e/o attuali di sofferenza e inferiorità consente
all’analista sempre (e a maggior ragione in questi casi) di non colludere –
cercando di risarcire per interposta persona se stesso – col paziente che,
gravato da sentimenti simili, si aspetta un imperituro "risarcimento
danni" per le passate sofferenze.
In base alla mia esperienza non è sempre facile sopportare l’invidia del paziente
ed interpretarla perché, per invidia, egli "deve" dimostrare – anche
a costo di mantenere certi sintomi – che il terapeuta è un fallito o un
incapace. In questi casi è utile allora che quest’ultimo elabori anche il
proprio senso di colpa per avere quel benessere (fisico, affettivo, economico)
che il paziente non ha e/o non ha avuto.
I testi freudiani ci avvertono che protratte sofferenza infantili possono
concorrere a specifiche difficoltà nella strutturazione del Super-io. Ritengo
che responsabile ne sia l’impiego del meccanismo dell’identificazione con
l’aggressore. Ciò, unitamente all’ipertrofia dell’Io Ideale, consente
l’installarsi della patologia del carattere "in credito con la vita".

"Life owes me"
Some reflections about treating character pathology that can be connected
with long-lasting infantile suffering.

Starting from some of Freud’s clinical cases (The Wolf-Man, King Richard
III, Kaiser Wilhelm II) I have drawn a few useful indications both for the
treatment of transferential and counter-transferential dynamics that develop
during the analysis of patients with character problems – associated with
infantile sufferings and/or impairments due either to physical illnesses or to
adverse environmental conditions – and, more in general, for an understanding
of character pathology.

To become aware of his own affects of shame and envy, linked to infantile
and/or current situations of suffering and inferiority, always (and even more
so in such cases) prevents the analyst – in an attempt to indemnify himself
vicariously – from colluding with the patient who, burdened with such feelings,
is expecting an everlasting "compensation for damage" because of his
past sufferings.

In my experience, it is not always easy to tolerate the patient’s envy and
to interpret it because the patient, driven by envy, "has to"
demonstrate – even at the cost of maintaining some symptoms – that the analyst
is a failure or a bungler. Thus, it is helpful if the therapist works through
his own feeling of guilt because he enjoys a (physical, affective, financial)
wellbeing that the patient is not experiencing and/or has not experienced ever.

Freud’s writings warn us that long-lasting childhood sufferings can
contribute to specific difficulties in the structuring of the superego. I
believe that the recruitment of the mechanism of identification with the
aggressor can be held accountable for these difficulties. This, together with a
hypertrophy of the ideal ego, allows for the formation of character pathology
that can be summarized as "life owes me".

La Casa del
Sé, la Casa del Tè: riflessioni su setting psicoanalitico e rito, tra occidente
e oriente.

Il setting
psicoanalitico, inteso come l’insieme delle costanti che configurano il
processo analitico, attiva una particolare disposizione materna dell’analista
all’ascolto delle comunicazioni verbali e non verbali del paziente.
La funzione di contenitore svolta dal setting, attraverso la stabilità del
luogo, del tempo e delle caratteristiche dell’incontro, favorisce il dipanarsi
della relazione verso la sua dimensione embrionale, fino ai livelli più
primitivi.
Nel ripetersi ritmico delle esperienze di presenza ed assenza, di andare e
venire, il rituale del setting ricalca i vissuti primitivi del neonato con la
madre e favorisce l’ integrazione di parti scisse.
Le autrici prenderanno in esame il senso profondo di questa ritualità e la
confronteranno con il rituale antico della Cerimonia del Tè in Giappone,
attraverso cui il cerimoniere si prende cura del suo ospite, promuovendone il
benessere, in una realtà altamente simbolica che permette un’esperienza di
illusione creativa, depositaria di possibilità trasformative.
Si metterà in luce il valore fortemente simbolico insito nella ritualità della
cornice analitica e l’importanza della fattualità terapeutica quotidiana che
attraverso la dimensione di contenimento offerta dall’analista consente la
costruzione della fiducia di base.
Verranno presentati due casi di pazienti con disturbi narcisistici ai quali
erano venute a mancare le cure materne e la stabilità affettiva necessaria per
consolidare relazioni affidabili.
I pazienti, entrambi di nazionalità italiana, sposando donne asiatiche hanno
cercato nel mondo orientale una realtà fortemente simbolica, sostenuta da riti
di passaggio, per recuperare un’appartenenza gruppale e dare voce ai loro
bisogni di identità.

Attraverso
la ritualità del setting analitico hanno ritrovato e restaurato il contatto con
un oggetto affidabile e caldo che ha permesso l’ incontro con le aree della
creatività e dell’illusione e l’avvio di relazioni affettive più mature.

Master of ceremonies, master of the Self: on
psychoanalytic setting and ritual, between West and East.

The psychoanalytic setting, intended as the constants which determine the
psychoanalytic process, activates a special maternal disposition of the analyst
to the listening of verbal and non verbal communication of his patient.
The function of container performed by the setting, through the stability of place,
time and the features of the meeting, favours the unravelling of the
relationship towards its embryonic dimension, to its most primitive levels.
Through the rhythmic repeating of the experience of presence and absence, of
going and coming, the ritual of the setting goes back to the primitive
experiences of the baby with the mother and favours the integration of split
parts.
The authors examine the deep sense of this rituality and compare it with the
ancient ritual of the Japanese Tea Ceremony through which the master of
ceremonies takes care of his guest, promoting his wellbeing, in a highly
symbolic situation which allows an experience of creative illusion, repository
of possibilities of transformation.
We highlight the deep symbolic value within the rituality of the analytic frame
and the importance of "daily therapeutic factuality" which allows the
building of basic confidence.

We introduce the case studies of two patients with narcissistic disorders
who lacked motherly care and the affective stability necessary to consolidate
reliable relationships.
Both Italian, they married Eastern women and looked into the Eastern world for
a highly symbolic reality, supported by passage rites, to recover a group
belonging, giving voice to their identity needs.

Through the rituality of the analytic setting, they could find and restore
the contact with a reliable, warm object which allowed the meeting with the
areas of creativity and illusion and the start of more mature affective
relationships.

Ascoltare l’inconscio: dall’area di rêverie alla simbolizzazione. Lavoro clinico con gli adolescenti

Il lavoro si propone di mettere in luce il legame che si instaura tra paziente e analista a partire dalle fantasie inconsce e da tutte quelle forme di comunicazione che si producono in modo occulto fin dai primi incontri.
L’analista accoglie gli aspetti inconsci del paziente e interagisce con essi, in una sorta di dialogo tra inconsci. L’analista accetta e cerca di riconoscere l’uso inconscio che il paziente ha necessità di fare e asseconda un processo in divenire, accettando l’incertezza e l’angoscia della perdita di parte della sua soggettività: attraverso la disponibilità dell’analista il Sé di entrambi entra in uno scambio empatico, creando una nuova esperienza nominata dalle autrici area di rêverie, intesa come un’area di scambio dentro la quale avvengono processi molto primitivi caratterizzati da proiezioni, introiezioni ed identificazioni proiettive.
Nell’area di rêverie paziente e analista creano possibilità di "sentire insieme", comporre e ricomporre la propria identità e nuove rappresentazioni di Sé che escono dalle rigidità .

Attraverso i casi clinici di due adolescenti verrà mostrato come la coppia analitica interagisca per mezzo di comunicazioni inconsce che penetrano dentro i confini di entrambi, modificandoli. In questo modo la coppia analitica diviene trasformatrice di memorie di esperienze traumatiche, quelle archiviate nella memoria implicita, in epoca preverbale.
Modi diversi di interagire, molti e diversi fattori sensoriali, verbali e non verbali, si intersecano e consentono la realizzazione di "eventi inconsci", vissuti nell’hic et nunc della seduta in relazione con l’analista e con la storia passata di entrambi.
Nei casi presentati la particolare attenzione alla relazione inconscia ha permesso di raggiungere parti nascoste e rifiutate del Sé dei pazienti e di attraversare insieme a loro quelle angosce di dispersione e frammentazione che hanno potuto trovare una via di trasformazione.

Listening to the unconsciuos: area of rêverie
and transformational relationships

This work intends to highlight the link which is established between patient
and therapist starting from the unconscious fantasies and from all the forms of
communication which are produced in a hidden way since the very beginning.
The analyst welcomes the unconscious aspects of the patient and interacts with
them, in a sort of dialogue between unconsciouses. The analyst accepts and
tries to acknowledge the unconscious use that the patient needs to make and
supports a process in transition, accepting the incertitude and the anguish of
loss of part of his/her subjectivity: through the flexibility availability of
the analyst the Self of both enters an empathic exchange, creating a new
experience named by the authors area of rêverie, intended as an exchange area
within which very primitive processes characterised by projections,
introjections and projective identifications.
In the area of rêverie patient and analyst create possibilities of
"feeling together", compose and re-compose their own identities and
new representations of the Self which are eased out of rigidities.

Through the case studies of two adolescents we will show how the analytic
couple interacts by means of unconscious communication which go deep into the
borders of both, modifying them. The analytic couple transforms memories of
traumatic experiences, recorded in the implied memory.
Different ways of interacting and several different sensorial factors,
intersect and allow the production of "unconscious events", lived in
the here and now of the session in relation with the analyst and with the past
history of both analyst and patient.
In the presented case studies the special attention to the unconscious
relationship allowed to reach hidden, refused parts of the Self of the patients
and to cross together with them dispersion and fragmentation anguishes, paving
the way to transformation.

GENERATIONS OF ANALYSTS, GENERATIONS OF
PATIENTS: NON-SYMBOLIZABLE DESTRUCTIVENESS IN CONTEMPORARY PATIENTS.

Abstract
Il presente lavoro si colloca in un’area di riflessioni sulla contemporaneità
dei disagi, con particolare attenzione alla distruttività e ad un’ipotesi di
lavoro per i pazienti con un deficit di simbolizzazione, alla luce della
frequente difficoltà di lavorare analiticamente con i pazienti contemporanei ed
alle domande sulla pertinenza relativa ai trattamenti analitici. Viene trattato
l’ argomento della mancanza di simbolizzazione come elemento favorente la
comparsa di distruttività senza contenitore.
In un caso clinico si riflette sulla transgenerazionalità e in particolare
sull’ importanza della terza generazione nello sviluppo psicopatologico di
stati mentali distruttivi.


Abstract
This paper posits itself within an area of reflection on the contemporary
nature of distress, in light of frequent difficulties in working analytically
with contemporary patients and of questions of pertinence related to analytic
treatments, paying particular attention to destructiveness and providing a
hypothesis for working with patients lacking symbolization.
Furthermore, this contribution places itself in a space that I imagine to be
before or around psychoanalysis, yet which presumes an analytic thought,
without which it would not have any sense.
This contribution starting from our way of working and living the relationship
with theory, meets the contemporary so called dependent patients. Regarding the
latter, I would consider the problem of dependence as one of the characteristic
themes of our times, both in terms of diffusiveness of the problem and its
functioning as a defense mechanism. Particularly important seems to be the
research in possible tools pertaining to analyzability of these kind of
contemporary patients, according to more levels, which at the same time imply a
psychoanalytic mind able to give up needing a relationship that the subject is
not yet able to sustain, and a research into intergenerational states.
In the paper these problems will be treated from the viewpoint of
destructiveness, desubjectivation, lack of symbolization as conditions that
increase the traumatic dimension of the mind, where the trauma that the subject
has, can grow from his or her own non-symbolizable destructiveness

ADDORMENTARSI
TRA PSICOANALISI E NEUROFISIOLOGIA: PRIMA DEL SOGNO

Partendo
dall’esame di alcune situazioni cliniche in cui si evidenzia una confusione tra
i modelli mentali verbalizzati e i modelli dell’azione, l’autore si propone di
esaminare quel particolare momento della vita fisiologica che è
l’addormentamento: stato di transizione tra la veglia e il sonno caratterizzato
da alcuni fenomeni psicologici,tipici della confusione tra processo percettivo
e processo rappresentativo con conseguente instabilità nell’esame di realtà, e
da altri riguardanti il comportamento muscolare.
La prima vignetta clinica si riferisce a quello stato di coscienza liminare o
transazionale in cui una paziente fatica a discernere tra costruzioni psichiche
inconsce e percezione da sveglia.
La seconda si riferisce sempre a quella fase di frammentazione che il pensiero
subisce durante l’addormentamento, ma le esperienze regressive vengono
esaminate alla luce del rapporto tra funzione simbolica del sogno e acting out.
Tali situazioni cliniche offrono lo spunto per un tentativo di integrazione tra
concetti ormai classici della psicoanalisi come quello di apparato per sognare
i sogni, di contenitore del sogno, di sogni di tipo elaborativo e di tipo
evacuativo, e altri concetti della teoria della mente e dei suoi legami con il
substrato neurobiologico.
L’autore propone un raccordo epistemico tra i vari modelli e anche tra discipline
che si propongono di studiare o i processi (neurofisiologia) o i significati
(psicoanalisi) della materia onirica.

FALLING ASLEEP BETWEEN PSYCHOANALYSIS AND
NEUROPHYSIOLOGY:
BEFORE DREAMING

Starting with the examination of some clinical situations where confusion
between verbalized mental models and acting out models were shown, the author
examines that particular moment of our psychological life, that is, falling
asleep: a transitional state between waking and sleep characterized by some
psychological phenomena, typical of the confusion between the process of
perceiving and the process of representing with resulting instability in the
examination of reality, and others regarding muscle behavior.

The first clinical vignette refers to the liminal or transitional state of
consciousness where a patient struggles to discern between unconscious psychic
mental constructions and perceptions on waking.
The second refers to the process of fragmentation that thought suffers whilst
the patient is falling asleep. However, these regressive experiences are
examined in light of the relationship between the symbolic function of dreaming
and acting out.

Such clinical situations offer a starting point for an attempt at the
integration between concepts which are now classics in psychoanalysis, such as
the "apparatus for dreaming dreams", the "container of
dreams", "elaborative and evacuative type dreams" and other
concepts of the theory of the mind and its links with the neurobiological substrate.

The author presents an epistemic connection between these various models
and also between disciplines which intend to study either the processes
(neurophysiology) or the meaning (psychoanalysis) of the material dream.

Passion as a therapeutical factor

Abstract
The use of passion in the psychoanalytical process has controversial and
important ramifications. Passion implies the experience of intensified
emotions, sublime-transient, and is the result of extraordinary affects
spiraling between positive and negative poles of instinct. Passion, in its full
expression, exists within the vitality of human experience in everything we
think and do.
Bion (1963) defines passion as the component derived from L, H, and K
experienced with intensity and warmth of thought without any suggestion of
violence."..Passion is evidence that two minds are linked." The
analyst focuses on the construction of the patient’s emotional mind and,
therefore, must also consider himself as an interlocutor within the patient’s
expressions of L, H, K. The evolution of potentially painful feelings, which
Bion calls premonitory-feelings, must be tolerated by both patient and analyst.

Confronted by an adolescent boy devoid of vitality who existed within a
sadistic fantasy world, the analyst created in her mind a fertile ground upon
which the patient was able to make contact. In this sadistic fantasy world
where he was omnipotent and invincible, death and mutilation were exciting for
him.
Once contact was made, two minds were linked and a passionate space was
created. The analyst’s capacity to receive and elaborate the unrepresented
emotions of the patient, gave the patient a sensibility through which he was
able to reach his own tolerable truth.

La passione sembra avere controversi significati nella relazione psicoanalitica
e, talvolta, rappresenterebbe più una forma di elusione dell’amare o del patire
che non il suo contrario.
Alimentata da fantasie, desideri e memorie, la passione conterrebbe in sé una
certa ambivalenza e sembrerebbe essere il risultato dell’intensificarsi di
affetti legati sia a qualcosa di sublime e straordinario ma anche di terribile
e catastrofico.
Bion(1963) sottolinea, invece, come gli elementi della seduta psicoanalitica
dovrebbero possedere, nello specifico, le dimensioni del senso, del mito e
della passione e aggiunge che"..la passione è l’evidenza che due menti
sono legate". La passione sembrerebbe riferirsi, dunque, allo stato
emotivo fluttuante dell’analista in risonanza alle emozioni del paziente. L’analista
si avvarrà delle sue proprie osservazioni sul paziente, delle fantasie inconsce
e della passione, cioè la capacità di rappresentare emotivamente le sue proprie
emozioni suscitate da quelle del paziente.
L’esperienza della sofferenza emotiva, dunque, permetterà all’analista di poter
formulare un’interpretazione adatta a quel paziente a sentirsi sufficientemente
al sicuro per esperire le proprie emozioni. Patire il dolore emotivo con la
fiducia di poter apprendere da quella esperienza il proprio significato personale.
In questo lavoro, verrà descritta l’analisi di un giovane adolescente
devitalizzato e deumanizzato, ritirato in un mondo immaginario violento e
sadico, dove la morte e la mutilazione erano motivo di eccitamento. In questo
ambiente deumanizzato, si è cercato di evidenziare quei momenti di particolare
forza passionale che hanno reso possibile avviare delle trasformazioni.

Abstract

In questo lavoro l’autore descrive l’incontro con le problematiche psichiche presentate dalla realtà delle adozioni, realtà sempre più presente nella società italiana. Le problematiche dell’adozione sono molto complesse, sia per la gravità della patologia a cui portano, la quale si situa nell’ambito dell’identità personale e familiare, sia per la varietà delle implicazioni teoriche, cliniche e tecniche che comportano.
Le situazioni familiari mettono a contatto con una sofferenza profonda e diffusa in cui emerge soprattutto la difficoltà sia nel bambino, che nei genitori, all’incontro con "lo straniero". Si esamina il vissuto dei genitori rispetto alla impossibilità di generare un figlio naturale. E nel bambino si descrive l’esperienza della mancanza di un ambiente "abbastanza buono", di rottura della continuità della vita, dell’abbandono e del contatto con il nuovo ambiente di adozione vissuto come "straniero". Intorno a tale tema si colloca la riflessione psicoanalitica dell’autore, tesa ad individuare le modalità, lo spazio e il tempo per aiutare i protagonisti a creare un lingua affettiva comune, capace di colmare la distanza tra suoni dissonanti e incomprensibili.
Ci si chiede come avviare e procedere in questo lavoro reciproco di riconoscimento, di conoscenza e di traduzione. E come far diventare tale incontro una esperienza di amore e di crescita psichica per tutti i membri della famiglia.
Nell’esempio clinico si è cercato di mostrare come possa funzionare, o essere utile per un certo
tempo , l’osservazione psicoanalitica condivisa, la quale consente di aiutare i genitori a
familiarizzare con il nuovo ruolo e il bambino a sentirsi accolto nella sua "diversità" e contenuto
in un ambiente "facilitante".
É da considerare infine che l’osservazione psicoanalitica condivisa consente anche allo psicoanalista di tenere a mente e lavorare considerando contemporaneamente tutte le componenti presenti, attingendo a livello della teoria psicoanalitica ad una metapsicologia che renda ragione di uno spazio psichico intrapsichico, interpersonale e gruppale.


Abstract


In this paper the author describes working with the mental problems found in adoptions, a reality ever more present in Italian society. The problems of adoptions are very complex, both for the pathology they result in, set in the sphere of personal and family identities, and for the variety of implications they carry: theoretical, clinical and technical.
The family situations bring profound, widespread suffering which reveals mainly the child’s and the parents’ difficulties in coming to terms with the "foreigner." The author examines the inner life of the parents in relation to the impossibility of generating a natural child. As for the child, the author describes how the child experiences the absence of a "good enough environment, the interruption of continuity in her life, abandonment, and the coming into contact with the new adoptive environment which the child undergoes as a "foreigner." The author places her psychoanalytical reflections on these themes, and aims at identifying the manner, the space, and the time need to help the protagonists to create a common affective language capable of bridging the gap between discordant, incomprehensible sounds.
The author raises the question of how to proceed in this work of mutual recognition, acquaintance and translation. And how to turn this coming together into an experience of love, and of mental growth for all the members of the family.
In the clinical example, the author tries to show how shared psychoanalytical observation may work, or be useful for a certain amount of time. It can allow the parents to be helped in becoming familiar with their new role, and it can help the child to feel received with all her "diversity," and made to feel at home in a "facilitating" environment.
Finally, the author points out that shared psychoanalytical observation also allows the psychoanalyst to bear in mind, and to work while considering contemporarily all of the components present. From psychoanalytical theory, a meta-psychology can thus be drawn, which will justify a mental, inter-psychic, interpersonal, and group space.

Il concetto di «modo operativo primario» del pensiero e la descrizione delle
sue regole costituiscono l’acquisizione più importante della psicoanalisi e il
fondamento del suo metodo: grazie a quella descrizione, a quel sistema
dettagliato di regole di trasformazione – logiche, temporali, spaziali e
linguistiche – la psicoanalisi poté costituirsi come un metodo per l’indagine
dei processi psichici inconsci e come una pratica clinica, quali aspetti di uno
stesso percorso.
Per via del triplice statuto della psicoanalisi – metodo d’indagine, teoria
psicologica e tecnica terapeutica – la via della conoscenza e la via della cura
sono la medesima via: le osservazioni e le interpretazioni psicoanalitiche
richiedono pertanto il concorso delle tre dimensioni e non tollerano la
sottrazione di alcuna di esse.
La teoria dell’inconscio e il metodo della cura costituiscono il «manico» e la
«lama» del coltello di Freud, in mancanza dei quali la psicoanalisi corre oggi
il rischio della sua afanisi da parte dei suoi stessi seguaci; nonché il
rischio della rinuncia a ogni criterio di verità scientifica. Freud non si
sottrasse al compito scientifico di ricerca della verità, con i suoi corollari,
il dolore e la disillusione; ma non rinunziò mai alla ricerca della felicità
possibile, indicandola nella capacità di annettere all’ideale le espressioni
della sessualità e nell’indispensabilità di un suo soddisfacimento diretto,
contro ogni presunta superiorità dell’uomo simbolico e delle sue produzioni disincarnate.

Freud’s knife – Abstract
The concept of the "primary operational mode" of thought and the
description of its rules are the paramount achievements of psychoanalysis and
the foundation of its method. By virtue of this description and this detailed
system of rules of logical, temporal, spatial and linguistic transformation,
psychoanalysis succeeded in establishing itself both as a method for
investigating unconscious psychic processes and as a clinical procedure, these
being two sides of one and the same coin.
Owing to the threefold status of psychoanalysis – as an investigative method, a
psychological theory and a therapeutic technique – the path of knowledge and
that of treatment are one and the same. Psychoanalytic observation and
interpretation thus call for the combination of all three elements and cannot
tolerate the removal of any one of them.
The theory of the unconscious and the method of treatment are the
"handle" and "blade" of Freud’s knife. Psychoanalysis runs
today the risk of aphania due to the abolition of its essential components by
its very adherents, as well as that of the abandonment of every standard of
scientific truth. Freud did not flinch from the scientific task of seeking
truth, with its corollaries of pain and disillusionment. However, he never gave
up the quest for the possibility of happiness, which he considered to lie in
the capacity to include expressions of sexuality and the indispensability of
its direct satisfaction within the purview of the ideal, rather than invoking
the presumed superiority of symbolic man and his disembodied productions.

Abstrac to  the intervention of Paolo Fonda:
I shall briefly review some aspects of the early psychic development,
pre-oedipal, in order to stress the traumatic effects of early separation. I
would like to emphasize the magnitude of the role played by fusion between the
child and the caregiving-object. It is inversely proportional to the importance
of separation that evolves during the maturation process. The child is
particularly vulnerable to early separations because of the indispensable
initial pervasiveness of fusion in his relationships.
